Counseling Services

CoachHorse is open year round for individual or group sessions. Scheduling is done by calling our office at 920.980.5326. We also recommend that you check our website for scheduled events.
Appointments
Appointments can be scheduled by calling 920 980 5326. A standard weekly session is 50 minutes in length although some sessions may be longer. Please give a 24-hour advance notice for cancellations. With less notice you will be billed for the time set aside for your use unless you and your professional counselor both agree that you were unable to attend due to circumstances beyond your control.
How are services paid for?
A standard individual weekly session is $80.00. If no insurance, services are billed on an ability to pay basis using a sliding fee scale. If the sessions are longer or shorter the fee may be adjusted accordingly. Cash or checks are accepted.
Insurance Policy
CoachHorse Counseling Services operates as an out-of-network provider giving clients all the necessary paperwork and receipts to file with their own insurance carrier for reimbursements. After you meet your deductible, most insurance companies will pay a percentage of each session. It is highly recommended that you call your insurance carrier to obtain information on what is covered before attending a session. CoachHorse Counseling Services is not a member of any in net-work insurance panels. In addition, if CoachHorse were a member, an official client diagnosis and an ongoing progress report, treatment plan, etc would have to be submitted which can be
available to others. Full payment will be due each session.
Confidentiality
Under Wisconsin law Sec. 51.30, Stats, all treatment information disclosed within sessions and records pertaining to the sessions are confidential and may not be released to anyone without an informed written consent of the individual, if competent, or the guardian. The rule covers both verbal information and treatment records.
There are exceptions to the above rule, where certain, specific people or entities may access certain information and/or records for certain purposes without the individual’s or guardian’s written consent.

Please note that if there is reasonable suspicion and it is necessary to attempt to protect others from harm, legally the professional counselor is obligated to take action. Some examples are when there is reason to believe that a child has been abused, a disabled adult or elder person has had a physical injury inflicted upon such disabled adult or elder person, other than by accidental means, or when you are believed to be in danger of hurting yourself, the rule will be broken. Every effort will be made with you to discuss the situation before breaking the rule. |
